Equilibrium Diagnostics

Developing a urine test to assess kidney function and predict disease progression

BiomarkersMedical DevicesPreventive Health

Chronic kidney disease can progress for years without symptoms. Routine assessment relies mainly on estimated glomerular filtration rate () and urine albumin, which capture filtration and glomerular damage but not the kidney tubules’ acid-handling function.

Equilibrium Diagnostics is developing the AP Reader and urine Ammonium–pH Index (uAPI), a urine-based test designed to measure and help identify patients at higher risk of disease progression. The test is intended to complement existing kidney markers such as eGFR and urine albumin.

Key Investor Signals

Secured DKK 4.2M through BII Venture Lab in 2026

Equilibrium Diagnostics was selected for ’s 2026 Venture Lab Cohort 10, which provides DKK 4.2M in convertible financing and 12 months of venture-building support.

uAPI added prognostic value across multiple patient cohorts

A 2026 preprint validated uAPI in three independent chronic kidney disease cohorts and additional type 2 diabetes cohorts, adding prognostic information beyond established kidney-risk models.

Patent-backed technology from Aarhus University

The company is built on Aarhus University-developed technology covered by patent applications for the urine analyzer and its use in predicting kidney disease progression.

Other Positive Signals

  • A 2024 peer-reviewed study linked lower urine acid-base scores to faster decline across three CKD cohorts.
  • awarded support for German validation and EU regulatory preparation.
  • A DKK 1.6M Augustinus Foundation grant funded international validation of the urine acid/base .
  • Currently offers urine acid-base assessment services for research studies.
  • A 2025 Nordic health-economics model estimated €75M in annual Danish savings from uAPI-based CKD risk stratification.

Product

Equilibrium Diagnostics is developing a urine-based kidney diagnostic built around the AP Reader and the urine Ammonium–pH Index (uAPI). The AP Reader measures urinary ammonium and pH from a single sample, and uAPI combines those measurements into a score designed to complement eGFR and albuminuria; the current AP Reader is positioned for research laboratories.

Measuring kidney tubular function directly

The AP Reader measures urinary ammonium and pH to assess how well the kidney tubules handle acid, adding information that standard kidney filtration and albumin-leakage measures do not directly capture.

Automated urine testing workflow

The AP Reader is an automated laboratory analyser designed to measure the two urine parameters reproducibly and fit into research and clinical laboratory workflows.

A single score for progression risk

uAPI combines urinary ammonium and pH into one score. Published CKD research links lower scores to faster progression, supporting its development for risk stratification and monitoring alongside standard kidney tests.

Market Overview

Chronic kidney disease often progresses without symptoms and remains widely undiagnosed. More than 850M people are estimated to be affected worldwide. In the U.S., over 35M adults have CKD, yet around 9 in 10 do not know they have it. This diagnostic gap can delay risk assessment, monitoring and referral.

Kidney-care guidelines use eGFR, urine albumin and validated kidney-failure risk equations to guide monitoring and specialist referral. The underlying tests primarily reflect filtration and glomerular damage. Equilibrium Diagnostics is developing a complementary urine test that adds information on tubular function and progression risk.
